    3. Defendant failed to pay
       (a) said note when due; or
       (b) the installment payment due on said note on .....(date)....., and plaintiff (has/has not) elected to accelerate payment of the balance.

    WHEREFORE, plaintiff demands judgment for damages against defendant.
